What are the Registers/Forms to be maintained by the establishment?

1. register in Form A showing the computation of allocable surplus

2. A register in Form B showing set-on and set-off of the allocable surplus.

3. A register in Form C showing the amount of bonus due, the deductions, and

actual amount of bonus disbursed.

4. The Annual return in Form D to be filed before February 1 st of each year.
